Local Assistance Program Guidelines (LAPG) forms 22N/22O and 23N/23O have been replaced by the new CTC Allocation Form. The old forms have been archived and taken off the Local Assistance website.
Starting with the January 30/31, 2019 California Transportation Commission (CTC) meeting, please only use the new CTC Allocation Form. This new form and all LAPG forms can be found on the LAPG Forms Webpage.
If you have already submitted an allocation request to your District Local Assistance Engineer (DLAE) using the old form, it will be accepted this time around. For future allocation requests, however, only the New CTC Allocation Form will be accepted.
Please note that the new form will only open directly upon clicking the link when using Internet Explorer. If using Chrome or another browser, you must first right-click, save to your computer, then open the file in Adobe PDF.
