Implementation of New LAPM Form 3-A, Project Authorization/Adjustment Request

Caltrans Division of Local Assistance released Office Bulletin DLA-OB 18-03 on November 29, 2018, introducing a new dynamic form, LAPM 3-A, Project Authorization/Adjustment Request, to request authorizations and adjustments for federal-aid projects. This form was created to streamline the request process and consolidate multiple forms into one dynamic form.

Mandatory Use of LAPM 3-A after April 1, 2019

Beginning April 1, 2019, project sponsors must use LAPM 3-A to request authorizations and adjustments for their non-emergency relief federal-aid projects.  On the LAPM Forms page of our Local Assistance website, we have rolled out LAPM 3-A and have relegated the prior duplicative forms to the “Superseded by LAPM 3-A” section in the right column. These prior forms continue to be made available to ensure our local and regional agencies don’t get caught in the middle of their Requests for Authorization process; however, our current plan is to remove these prior forms from the LAPM Forms page and archive them by June 1. As of July 1, Requests for Authorizations received using the prior forms will be returned to the project sponsor for resubmittal using LAPM 3-A.
