The Caltrans Headquarters Office of Civil Rights is requesting the below DBE-related exhibits for fiscal year 2022-23 to be submitted by October 15, 2023. Collection of these forms and respective data is required to compile the Caltrans Uniform Data Report, which is due annually to the Federal Highway Administration in mid-October.
Note: If already submitted, these exhibits do not need to be resent.

• Exhibit 9-P: Prompt Payment Certification (Excel)
• Exhibit 10-O2: Consultant Contract DBE Commitment
• Exhibit 15-G: Construction Contract DBE Commitment
• Exhibit 17-F: Final Report – Utilization of Disadvantaged Business Enterprises and First-Tier Subcontractors
